HOW CAN I BE ASSESSED BY A SLEEP LABORATORY?
If you think you may have a sleep disorder, consult your doctor. Referrals to this set of sleep laboratories is by physician referral only. This includes physicians such as your family doctor or other medical specialists. If your doctor suspects you may have a sleep disorder, he/she is required to complete and sign a sleep study requisition form which can be faxed or mailed to the sleep laboratory. Your physician may obtain a referral form from this website or by calling our sleep laboratories. If your doctor refers you to one of our sleep laboratories for diagnostic assessment, please use the patient information section of this website (select from main menu) to answer any questions that you may have concerning how sleep studies are performed.
